We are looking for an experienced Hospice RN to join our central Texas team to provide start of cares and case management for our senior patients in their place of residence. Your role will be a Halcyon Hospice clinical provider to coordinate services, provide care, and prepare clinical documentation for your caseload, with the ultimate goal of providing effective communication to patients, family members, team, and other healthcare professionals.

Hospice experience is essential, along with a proven ability to manage a fast-paced, independent work schedule. Knowledge of senior living communities and WellSky/Kinnser software would be an asset!

Requirements

Must Have:

Active state RN License, with minimum one year nursing experience

Hospice experience

Covid vaccinated

Proof of current CPR

Reliable transportation and valid auto liability insurance

Good interpersonal skills

Computer literacy
